Quantity dispensed cannot exceed allowable amount by patient’s insurance plan.

If quantity ordered is less than allowable amount, the quantity ordered will be sent.

If quantity ordered is greater then the allowable amount, the max quantity allowed by insurance will be sent. 

Quattro FX frame is not universal, make sure to pick the correct cushion for your frame.
